NDIS Participant Privacy & Service Information Notice

Information for participants, nominees, guardians, plan managers and support coordinators.

1. Our NDIS-related services

AfroFusionPlate provides NDIS-related supports and services within its current provider registration and service scope. Website services include practical cooking and independent-living skills through AfroFusionPlate Academy, meal-preparation related supports and nutritional meal services where appropriate and within applicable NDIS arrangements.

2. Funding and claimability

NDIS funding is individual. Whether a support can be paid from a participant’s plan depends on the participant’s plan, goals, funding type, applicable NDIS pricing and claiming rules, the support actually delivered, and any service agreement or plan-manager requirements. Not every retail meal, catering purchase or Academy activity is automatically NDIS funded.

3. Information we may need

  • participant and authorised representative contact details;
  • NDIS number and plan/funding information where necessary for service administration or claiming;
  • plan manager, support coordinator or nominee details;
  • goals, support needs, learning preferences and accessibility requirements;
  • dietary, allergy, sensory, health or disability information reasonably necessary to provide safe and suitable services;
  • service notes, attendance, invoices and records required for service delivery, quality, complaints or compliance.

4. Consent and choice

Where sensitive information is collected, we seek consent or rely on another lawful basis. Participants should be involved in decisions about their services to the greatest extent possible. A participant may have a nominee, guardian, advocate or other authorised person involved where appropriate.

5. Rights, dignity and respect

We aim to provide supports in a way that respects individual rights, privacy, dignity, culture, identity, communication preferences and personal choice. We do not tolerate retaliation against a person for raising a concern or complaint.

6. Service agreements and cancellation

Where an NDIS service agreement applies, it should identify the supports, prices, responsibilities, cancellation arrangements, invoicing/claiming method, complaints process and any participant-specific requirements. If a service agreement conflicts with a general website term, the service-specific agreement applies to the extent of the conflict, subject to law.

7. Complaints and external escalation

Participants, family members, carers, advocates and representatives can raise concerns directly with AfroFusionPlate. Concerns about the quality or safety of NDIS supports may also be raised with the NDIS Quality and Safeguards Commission. A participant may seek support from an advocate when making a complaint.

8. Privacy

NDIS and health/disability information is handled as sensitive information under our Privacy Policy. We only disclose information where reasonably necessary for service delivery, claiming, plan management, safety, legal obligations or with consent, as permitted by law.
